Researchers reported at the American Physiological Society’s Experimental Biology 2010′s conference (April 24-28) that the stage for osteoporosis is set before menopause but exercise can forestall it. It is known that decreasing estrogen levels are associated with osteoporosis but it is now believed that bone density starts to decline years before this according to Joseph Cannon, Keller Chair in Allied Health Sciences and principal investigator of the National Institute of Aging-funded study .
Cannon hypothesized that higher levels of follicle-stimulating hormone (FSH) decrease bone mineral density by influencing the production of cytokines (regulatory proteins produced by white blood cells). The researchers were especially interested in one cytokine, interleukin-1, that signals certain cells to transform into osteoclasts that break down and resorb bone.
They measured FSH and bone mineral density in 36 women between ages 20 and 50 and correlated higher FSH with lower bone density. Incubating FSH with white blood cells isolated from the women, interleukin-1 was stimulated and higher circulating levels of interleukin-1 was correlated with lower bone mineral density. In addition, subjects who exercised more than 180 minutes a week had greater bone density. Thus, the study provided more evidence that exercise is important to maintain bone density and seems to promote inhibiting factors that keep interleukin-1 and bone breakdown under control. The researchers plan more research to determine the mechanism of how exercise influences interleukin-1.

Alexander Technique Effective for Back Pain
Wednesday, March 24th, 2010In a study published in Family Practice researchers concluded that the Alexander technique was considered effective by most patients. The Alexander Technique is a method of teaching bodily awareness and reducing muscle tension. Patients are taught standing and sitting postures and how to move correctly.
Researchers at the University of Southampton and Bristol surveyed patients to determine their expectations of and experiences with the Alexander technique and prescribed exercises. One hundred eighty three people who had received lessons in the Alexander technique returned questionnaires and 176 who had received prescribed exercises returned them. A follow up questionnaire was completed 3 months after the first and many of the subjects were interviewed. Findings included positive expectations of both groups at the beginning of the study that remained the same for the exercise group after 3 months. However, the expectations were higher after 3 months for the Alexander technique group. This group believed they had learned how to manage their back pain and the method fit well into their lifestyle.

Dr Vincent is a graduate of the Chiropractic Institute of New York and is currently the President of Integrated Health Care Practice Resources and past president of the Practice Resource Group, a management organization. He is also past president of the Society of Chiropractic Management Consultants and also served as the president and CEO of Spine Management Care Network and as clinical director of Chiropractic Services for Triad Health Care, Inc.
Dr Vincent introduced the Graston Technique, a form of instrument-assisted soft tissue mobilization to the Chiropractic profession in 2001. Strength Training, Self-Management Programs or a Combination are Equally Effective for Osteoarthritis
Wednesday, February 3rd, 2010A 24 month unblinded, random intervention study to compare the effects of strength training programs, self-management programs, and a combination of both carried out at the University of Arizona Arthritis Center found that physically inactive, middle aged people with symptomatic osteoarthritis benefited equally from strength training regimes, self-management programs, or a combination of the two. In this study, known as the Knee Study the researchers hypothesized that combining the two treatments might enhance the outcome.
Two hundred seventy three study participants between the ages of 35 and 65 who had reported pain and disability due to knee pain on most days in one or both knees for a period of not more than five years, and had a Kellgren/Lawrence classification grade 2 radiographic evidence of knee osteoarthritis in one or both knees were included.
Study participants were randomly assigned to 1 of 3 treatment groups-strength training, self management program, and the combination group. The strength training group spent the first 9 months improving the core areas of stretching and balance, range of motion and flexibility, and isotonic muscle strength. This was followed by 15 months of developing independent, long-term exercise habits. The second group participated in a 2-phase self-management program to educate participants and provide one on one treatment advice. The combined group participated in both the complete training and self-management program. Two hundred one of the original 273 participants completed the two year program with the self management group having the highest compliance rate.
Despite the lack of differences in finding in the three groups, all groups demonstrated improvements in physical function tests and decreased self-reported pain and disability. The researchers stated that because the self management group demonstrated higher compliance and there was no differences found in the outcome of the three groups, self-management might be a less intrusive and equally effective early treatment for knee osteoarthritis. Prostate Cancer Patients May Reduce the Risk of Death with Exercise A New Study Reports
Friday, December 18th, 2009A study presented at the American Association for Cancer Research Frontiers in Cancer Prevention Research Conference from December 6 to 9 by researchers from the Harvard School of Public Health reported that men with prostate cancer
who do physical activity as little as 15 minutes a day can reduce their overall mortality rates. They looked at both overall survival of men with prostate cancer and death rates specific to prostate cancer. They concluded that a moderate amount of regular exercise may improved overall survival and 5 or more hours a week of vigorous exercise may reduce the risk of death from prostate cancer.
Physical activity of 2,686 men enrolled in the Health Professionals Follow-Up Study before and after a diagnoses of prostrate cancer (men with metastases when diagnosed were excluded) was assessed. Men who participated in biking, swimming, jogging, or playing tennis about a half hour weekly had a 35% lower risk of overall mortality. Men who walked 4 or more hours weekly had a 23% lower risk of all-causes of mortality compared to men who walked less than 20 minutes a week. However, walking did not show an effect on specific prostate cancer mortality but strenuous exercise did and men who participated in 5 or more hours of vigorous physical activity weekly had a decreased risk of dying from prostate cancer.
Although the specific mechanisms of exercise on prostate cancer are unknown, the researchers said exercise is known to influence a number of hormones hypothesized to stimulate prostate cancer, improve immune functioning, and reduce inflammation. The interaction of these factors is still being. studied and more research is forthcoming.
